Registration Process

In an effort to comply with USASA in putting our state registration database online, below you can download important information to help start the process. One attachment is a sample Excel spreadsheet, which displays the 9 required columns.

Those columns are:

  • Team Name,
  • First Name,
  • Last Name,
  • Birth Date,
  • Gender,
  • Address,
  • City,
  • State, and
  • Zip Code
The second attachment lists these columns and optional columns and the desired formatting for all, including the maximum number of characters for each.

Please do your best to conform to this format. Do not combine information in one column. Each piece of information should have it's own column. Feel free to include additional columns to suit your league's needs. If you have questions or need assistance with your database, please let me know.
